What are the responsibilities and job description for the Family Nurse Practitioner (FNP) position at PRIMARY HEALTH SERVICES CENTER?
Job Summary:
The Nurse Practitioner works as part of the clinic provider team performing a variety of direct patient services including assessment, diagnosis, implementation of treat, evaluation, and monitoring the general patient management plan for clinic patients. The Nurse Practitioner performs these functions in accordance with the collaborative practice agreement (CPA) with the collaborating physician in the primary health care setting.
Qualifications:
1. Must have a current unencumbered Louisiana APRN license.
2. Must have graduated from a nationally accredited nurse practitioner program.
3. Must have national nurse practitioner certification in area of specialty.
4. Must have sufficient experience to carry out the duties of this position.
5. Must have a current Basic Life Support Card.
6. Must have written and oral communication skills.
7. Must be able to withstand 8-12 hours or more of standing, walking, stooping, bending, and sitting.
8. Must be able to handle a fast pace environment.
9. Must have sufficient experience to carry out duties of this position.

Responsibilities:
A. Attendance
1. Regular and punctual attendance. Individual is a workstation when scheduled and remains for duration of shift.
2. Notifies Medical Director and/or COO if any avoidable absence as soon as possible.
B. Performance
1. Obtains complete medical history from patient, and/or family, and/or previous medical record, etc.
2. Performs complete physical examinations on patients.
3. Assesses, diagnoses, implements treatment, evaluates, and monitors patients’ health status.
4. Orders routine and diagnostic lab and x-ray studies according to CPA.
5. Diagnoses acute and chronic illness on basis of clinical findings, laboratory and x-ray. reports and /or results of other diagnostic procedures according to approved protocols.
6. Formulates, initiates and monitors patient management plans for patients assigned to the Nurse Practitioner, using protocols and/or consultation with a collaborating physician.
7. Prescribes medications according to protocols and therapeutic goals and explains, treatment to patients and families.
8. Teaches and counsels patients and families.
9. Refers patients requiring hospitalization or assessment and management of conditions to collaborating physicians or appropriate specialist.
10. Follows patient’s progress with collaborating physician.
11. Documents all care provided on the EHR in a timely manner.
12. Renders emergency care including cardiopulmonary resuscitation, starting IV fluids and medications according to protocols.
C. Productivity
1. Maintains records-completely, adequately and with timely documentation.
2. Documents visits including physical examination, diagnosis, treatment, medications, etc.
D. Safety
1. Adheres to all clinic infection control, safety, and hazardous waste policies.
2. Reports any safety and or OSH violations promptly.
E. Team Work
1. Consults with physicians and other team members of the health care team as necessary.
2. Performs other necessary duties as required by Primary Health Services Center to meet the goal of providing primary health care services.
3. Works with all clinic staff to maintain a safe and therapeutic environment for patients, staff and visitors.
4. Performs other necessary duties as required by the Clinical Supervisor, MD, CEO or COO to meet the goal of providing primary health care services.
5. Participates in the Primary Health Services Center Quality Assurance activities and performs duties in accordance with applicable standards.
6. Comply with applicable law and fully compliant and cooperate with PHSC’s Compliance Program.
F. Professionalism
1. Maintains current unencumbered Louisiana Advance Practice Registered
Nurse (APRN) license.
2. Maintains current national certification in field of study.
3. Maintains current Basic Life Support certification.
4. Complies with organization policies and procedures.
5. Participates in PECS audit.
6. Adheres to company and clinical dress code.
7. Maintains confidentiality to patients, families, and fellow employees.
8. Follows HIPAA guidelines.
9. Accountable and responsible for self.
G. Performance Improvement
1. Maintains all patient records to comply with required standards.
2. Attends all medical staff meetings.
3. Attends all mandatory meetings.
4. Reviews clinical and administrative policies as needed.
H. Corporate Compliance
1. Will be complaint with applicable law.
2. Will be fully compliant and cooperate with PHSC’s Compliance Program.
3. Any other duties that may be assigned by supervisor, Medical Director, or management.
Evaluations
Formal evaluations will be completed by supervisor at 6 months and annually thereafter. It may be initiated at other times when deemed necessary.
