Job Overview

Duties
- Conduct thorough patient assessments including physical examinations, vital signs, and medical history reviews to establish accurate diagnoses.
- Develop and implement personalized treatment plans for acute illnesses, chronic conditions, and preventive care needs.
- Administer medications, including injections and IV infusions, following best practices and safety protocols.
- Utilize electronic health record (EHR) systems to document patient encounters accurately and efficiently.
- Coordinate care with specialists in areas such as gastroenterology, infectious disease care, geriatrics, hospice & palliative medicine, and behavioral health to optimize patient outcomes.
- Engage in telehealth consultations to extend access to care and support remote patient management.
Skills
- Proven experience working with diverse populations including children, seniors in assisted living or nursing homes, and individuals with developmental disabilities or dementia care needs.
- Strong knowledge of medical terminology, physiology, anatomy knowledge, CPT coding, ICD-10 coding standards for documentation and billing purposes.
- Proficiency in EMR/EHR systems for efficient patient documentation.
- Ability to triage effectively in urgent care settings; experience in hospital medicine or emergency medicine enhances your capability to handle complex cases.
- Knowledge of infection control practices; familiarity with HIPAA regulations ensures confidentiality of patient information.
- Excellent communication skills for health coaching and patient education.
