PRP Support Counselor

Primal Point Health is seeking a compassionate, motivated, and dependable PRP (Psychiatric Rehabilitation Program) Support Counselor to join our behavioral health team. The ideal candidate is passionate about helping individuals with mental health challenges develop the skills needed to improve daily functioning, independence, emotional wellness, and community integration.
Responsibilities
- Provide individualized PRP support services to clients in home, school, community, and office settings
- Assist clients with developing coping skills, emotional regulation, communication, socialization, and independent living skills
- Support treatment goals outlined in rehabilitation plans
- Maintain accurate and timely documentation in compliance with agency and state requirements
- Coordinate care with therapists, psychiatrists, case managers, schools, and family members as appropriate
- Conduct skill-building activities related to education, employment readiness, daily living, and community engagement
- Monitor client progress and report significant behavioral or emotional changes to supervisors
- Participate in team meetings, trainings, and supervision sessions
Qualifications
- High school diploma or GED required; Associate’s or Bachelor’s degree in Psychology, Social Work, Counseling, Human Services, or related field preferred
- Experience working with children, adolescents, or adults with mental health or behavioral health needs preferred
- Strong communication, organizational, and interpersonal skills
- Ability to work independently and manage community-based caseloads
- Reliable transportation and valid driver’s license required
- CPR/First Aid certification preferred
