Pride Health is seeking a RN Home Health for a 13-week contract (with high possibility of extension) at a healthcare facility in Lynchburg, VA. This is a great opportunity for experienced candidates to work in a fast-paced healthcare environment while gaining valuable experience.
Position Details:

- Hours per Week: 40 hours
- Pay Range: $2100 - $2300 weekly gross DOE
Key Responsibilities:
- Conduct comprehensive assessments of home health and hospice patients to identify physical, psychosocial, and environmental needs, and develop individualized Plans of Care accordingly.
- Deliver direct nursing care, including infection control and pain management, while documenting all interventions and patient progress to ensure continuity and quality of care.
- Educate patients and families on care routines, medications, diet, and home safety, tailoring teaching methods to individual learning needs and cultural backgrounds.
- Supervise and guide HHAs, STNAs, and LVNs to maintain high standards of service delivery and coordinate with physicians and interdisciplinary team members.
- Manage referrals, comply with third-party payer requirements, participate in on-call rotations, and attend care planning and team meetings as needed.
Requirements:
- Minimum 1 year recent experience in: Adult Home Health OR Hospice
- First-time travelers: Not accepted
- License: Active (must be in hand)
- Certifications: CPR, BLS
- Must have: Reliable vehicle, Valid driver’s license, Auto insurance
echnical Skills
- Required proficiency in: HomeCare HomeBase (HCHB) – within last 6 months
- OASIS (mandatory for RNs)
