Pride Health is seeking a RN Hospice/Home Health for a 13-week contract (with high possibility of extension) at a healthcare facility in Kannapolis, NC. This is a great opportunity for experienced candidates to work in a fast-paced healthcare environment while gaining valuable experience.
Position Details:

- Hours per Week: 40 hours
- Travel Pay: $45/hr - $51/hr
Key Responsibilities:
- Provide compassionate end-of-life care to hospice-eligible patients in personal homes, assisted living facilities, and skilled nursing facilities.
- Conduct 5–6 patient visits per day (avg. 25 per week), including assessments, symptom management, medication administration, catheter care, GT feeding, and pain control via CAD pumps.
- Educate patients and families on end-of-life care, expected decline, and available support resources; offer emotional and bereavement support.
- Document care, assessments, and IDG (Interdisciplinary Group) notes in Homecare Homebase to ensure compliance with hospice eligibility and continuity of care.
- Collaborate with the interdisciplinary team during biweekly case conferences to review patient status and update individualized care plans.
Experience:
- 2 years experience as an RN in a clinical setting, preferably in a home health or hospice setting
- Homecare Homebase Software Experience
- OASIS Experience
Certifications & Skills:
- Active RN License
- Valid Driver's License
- BLS (AHA)
