We are seeking an experienced Registered Nurse (RN) Hospice Case Manager to provide compassionate, patient-centered hospice care across various care settings. The RN Case Manager will coordinate and manage individualized care plans for patients with life-limiting illnesses while supporting families and caregivers through the end-of-life journey. This role requires strong clinical assessment skills, interdisciplinary collaboration, and a commitment to delivering high-quality hospice and palliative care.
Key Responsibilities
- Perform comprehensive physical, psychosocial, developmental, environmental, and safety assessments.
- Develop, implement, and revise individualized patient care plans based on identified needs.
- Assess and manage pain and symptoms, creating personalized comfort-focused treatment plans.
- Anticipate, prevent, and address undesirable symptoms and complications.
- Coordinate care and services for patients and families across the continuum of care.
- Participate in interdisciplinary team (IDT) meetings to establish, review, and update care plans.
- Maintain accurate, complete, and compliant electronic medical records.
- Educate patients, families, caregivers, and support staff regarding disease processes, medications, treatments, and symptom management.
- Provide holistic, family-centered care that supports quality of life and patient dignity.
- Support patients and families through end-of-life care, grief, bereavement, and emotional needs.
- Respect and incorporate patients' spiritual, cultural, and personal preferences into care delivery.
- Collaborate with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive care.
- Deliver care in accordance with hospice and palliative care standards and regulatory requirements.
Qualifications
- Graduate of an accredited Nursing program.
- Active Registered Nurse (RN) license in good standing.
- Previous Hospice Nursing experience required.
- Experience working as an RN Case Manager in a hospice setting.
- Proficiency with Homecare Homebase (HCHB) required.
- Strong knowledge of hospice and palliative care principles, symptom management, pain control, and end-of-life care practices.
- Skilled in case management, care coordination, and interdisciplinary team collaboration.
- Experience providing patient and family education, counseling, and advocacy.
- Strong clinical assessment, critical thinking, and documentation skills.
- Proficient with Electronic Medical Records (EMR) and regulatory compliance requirements.
- Excellent communication, interpersonal, and organizational abilities.

Important Details:
- 13-week contract position (extensions available)
- Shift is 8 hrs/day shift (9 AM - 5 PM), 40 hrs/week
- Location is Fort Worth, TX 76107
- Pay Range for Locals: $44 - $48 per hour
- Pay Range for Travelers: $2,140 - $2,180 per week
