Position Overview
Seeking an experienced Registered Nurse (RN) to provide direct patient care in a field-based home health setting in Moline, IL and surrounding areas. This role focuses on delivering high-quality in-home nursing services, completing documentation, and managing patient care independently. The ideal candidate has home health experience, is proficient in HomeCare HomeBase, and is comfortable working autonomously.
Schedule & Work Details
- Location: Moline, IL and surrounding areas
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours
- On-call/Weekend Requirement: Yes, participation in on-call rotation required
- Pay Rate: $51.00 to $53.00 per hour
- Mileage Reimbursement: Yes, between patient visits from first patient to last patient
Key Responsibilities
- Provide direct skilled nursing care to patients in their homes
- Perform comprehensive patient assessments and ongoing evaluations
- Complete accurate and timely documentation using HomeCare HomeBase
- Deliver care in accordance with individualized care plans and physician orders
- Monitor patient conditions and communicate changes to appropriate team members
- Educate patients and families on disease processes, medications, and care plans
- Ensure compliance with agency policies and regulatory standards
- Participate in on-call rotation and respond to patient needs as required
Required Qualifications
- Active Registered Nurse (RN) license
- Home health experience
- Experience with HomeCare HomeBase (HCHB)
- Strong clinical assessment and documentation skills
- Ability to work independently and manage a caseload
Preferred Qualifications
- OASIS experience and/or certification
- Strong organizational and time management skills
- Excellent communication and interpersonal abilities

- Ability to adapt to a fast-paced, field-based environment
