Position Overview
Seeking an experienced and highly motivated Registered Nurse (RN) to provide comprehensive home health services across multiple counties in Indiana. This role involves full-cycle case management, including patient admissions, OASIS assessments, supervision of clinical staff, and coordination of care. The ideal candidate is experienced in home health, proficient in HomeCare HomeBase, and committed to delivering high-quality, patient-centered care.
Schedule & Work Details
- Location: White, Pulaski, Fulton, Cass, Howard, Carroll, Miami, and Tipton counties
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 36 hours
- Productivity Expectation: 30 points per week
- On-call/Weekend Requirement:
- Every 4th weekend (Friday 4:30 PM to Monday 8:00 AM)
- 1 to 2 evenings per week (4:30 PM to 8:00 AM)
- Pay Rate: $51.00 to $53.00 per hour
- Mileage Reimbursement: Yes, between patient visits from first patient to last patient
Key Responsibilities
- Initiate, develop, implement, and revise individualized patient care plans in collaboration with physicians and the care team
- Perform comprehensive nursing assessments, including OASIS documentation and medication reconciliation
- Conduct admissions and determine patient eligibility for home health services
- Manage a caseload of patients and oversee ongoing care and outcomes
- Supervise and provide direction to LPNs and home health aides, including required supervisory visits
- Ensure delivery of high-quality care in accordance with infection control standards and regulatory requirements
- Accurately document all patient visits in compliance with agency and payer guidelines
- Communicate effectively with physicians, patients, families, and internal team members regarding care plans and changes in condition
- Participate in on-call rotation and weekend coverage as required
- Support performance improvement initiatives and survey readiness activities
- Assist with onboarding and mentoring of new staff when needed
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license in Indiana
- Current CPR certification
- Recent skilled home health experience
- Proficiency in HomeCare HomeBase (HCHB)
- Strong knowledge of OASIS documentation
- Understanding of Medicare home health guidelines
- Valid driver’s license, active auto insurance, and reliable transportation
