Position Overview
We are seeking an experienced Home Health Registered Nurse to deliver high-quality, patient-centered care in a field-based setting. This role is responsible for managing a caseload, performing comprehensive OASIS assessments, developing individualized plans of care, and collaborating with interdisciplinary teams to ensure positive patient outcomes. The ideal candidate brings strong clinical judgment, home health expertise, and the ability to work independently while maintaining compliance with regulatory standards.
Schedule & Work Details
Location: Coverage primarily in Wabash, Huntington, Grant, and Blackford counties, with additional coverage in Madison and Delaware counties as needed
Shift: Monday through Friday, 8:00 AM to 4:30 PM
On-Call/Weekend Requirements: Every third weekend (Friday 4:30 PM through Monday 8:00 AM) and one to two evenings per week (4:30 PM to 8:00 AM)
Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
Pay Rate: $51 to $53 per hour
Key Responsibilities
- Initiate, develop, implement, and revise individualized patient care plans in collaboration with physicians and the care team
- Perform comprehensive nursing assessments, including accurate and timely OASIS documentation and medication reconciliation
- Ensure patients meet home health eligibility and medical necessity guidelines and establish plan of care accordingly
- Provide skilled nursing care in accordance with infection control standards and plan of care
- Supervise and support home health aides and LPNs, including providing instruction and conducting required supervisory visits
- Document all patient visits in compliance with agency policy and payer requirements, ensuring timely sync per company standards
- Communicate effectively with physicians, team members, patients, and families to coordinate care and address patient needs
- Manage visit schedules, assignments, and care coordination while maintaining high quality standards
- Participate in performance improvement initiatives, survey readiness activities, and efforts to reduce hospitalizations and improve patient safety
- Assist with orientation of new staff and serve as a preceptor for team members and students
- Participate in on-call rotation and support agency utilization management processes
- Provide care in a variety of home environments, ensuring respect and professionalism in all situations
Required Qualifications
- Current and unrestricted Registered Nurse (RN) license in the state of Indiana
- Recent skilled home health experience
- Proficiency with HomeCare HomeBase (HCHB) and OASIS documentation
- Strong understanding of Medicare home health guidelines
- Current driver’s license, valid auto insurance, and access to a reliable vehicle
