Location: Osceola County, Florida

Schedule: Monday-Friday, 8 AM-5 PM
Travel: Required between 4 facilities during shift
Dress Code: Medical attire (no specific scrub color)

Position Overview

We are seeking a dedicated Registered Nurse (RN) to join our interdisciplinary care team as a Clinical Care Coordinator. In this role, you will support care coordination, clinical assessments, documentation quality, and continuity of care for members residing in facility-based settings. This position requires strong clinical judgment, independence, communication skills, and the ability to collaborate effectively with providers, advanced practice clinicians (APCs), facility teams, and family members.

Primary ResponsibilitiesCare Coordination & Clinical Support
  • Ensure accurate, timely, and complete documentation.
  • Communicate and collaborate with interdisciplinary care team members.
  • Assist the provider/team with a wide range of care coordination activities.
  • Support benefit determination for Medicare Part A, Part B, and Part D.
  • Partner with APCs and facility teams to identify and respond to patient changes in condition.
  • Assist with therapy coordination for members.
  • Participate in facility-based meetings and interdisciplinary huddles.
Patient, Family & Team Engagement
  • Schedule and participate in family conferences and team case presentations.
  • Provide education and support through in-services for selected facility teams.
  • Support members experiencing acute medical changes or chronic condition exacerbations (testing, diagnostics, interventions).
  • Assist in care management during short-term sub-acute rehabilitation episodes.
Quality & Documentation Support
  • Support training, tracking, and compliance with quality measures.
  • Assist with accurate documentation, coding, medication reconciliation, nursing rounds, assessments, and patient charting.
  • May write verbal orders from providers in compliance with state RN practice laws and facility policies.
  • Maintain thorough and accurate use of EMR systems and electronic documentation processes.
Required Qualifications
  • Active and unrestricted RN license in state of residence (or ability to obtain by start date).
  • Associate degree or higher in Nursing.
  • Certified in Basic Life Support (BLS).
  • Ability to meet physical job requirements, including lifting 30 lbs and performing physical assessments.
  • Access to reliable transportation for intra‑shift travel.
  • Full COVID‑19 vaccination (required by federal, state, and client regulations; documentation required).
  • Must pass pre-employment drug screening (UnitedHealth Group is a drug-free workplace).
Preferred Qualifications
  • 1 year of clinical experience (long-term care experience strongly preferred).
  • Experience supporting patients with complex medical, behavioral, or functional needs.
  • Strong understanding of geriatrics and chronic illness management.
  • Proficiency with electronic and written medical documentation.
  • Strong communication, organization, and time management skills.
  • Ability to work effectively with diverse care teams across multiple facility settings.
  • Ability to travel between multiple facilities during the workday.
Certifications Required
  • Active/unrestricted RN license.
  • Basic Life Support (BLS).
  • Associate degree or higher in Nursing.
