Pride Health is hiring a Travel Physical Therapist to support our client’s Home Health services in Renton, WA. This is a Contract opportunity for 3 months.
Job Details:
Job Name: Travel Physical Therapist - Home Health
Location: Renton, WA 98057
Duration: 3 months Contract
Payrate: $54 - $56/hr.
Schedule: Monday - Friday | 8:00 AM - 4:30 PM
Weekly Hours: 36-40 hours
Travel/Floating: 100% - must visit patients across South King County and may float to nearby LHC agencies
Job Responsibilities:
• Evaluate patients’ functional status, body systems, and physical therapy needs per CMS documentation
• Develop and implement individualized PT plans of care in coordination with physicians and healthcare team
• Observe, monitor, and report patient responses and progress to supervisors or physicians
• Treat patients to relieve pain, restore function, and maintain maximum functional independence using evidence-based techniques
• Educate patients, caregivers, and healthcare team members on plan of care, functional limitations, and progress toward goals
• Complete evaluations, progress notes, and discharge summaries according to LHC policies and state regulations
• Supervise PTAs and home health aides as permitted by state regulations and agency policy
• Participate in staff meetings, in-service education, and professional development
• Provide schedule flexibility, including potential weekend rotation or holiday coverage, to meet patient care needs
• Adhere to LHC Therapy Standards of Practice and maintain licensure and clinical knowledge up to date
• Perform all other duties as assigned
Requirements:
• Current Physical Therapy licensure in Washington or compact license granting practice privileges in WA
• Current CPR certification
• Driver’s license, vehicle insurance, and access to a reliable vehicle
• Bachelor’s Degree in Physical Therapy
• Strong communication, clinical judgment, and patient care skills
Nice to Have:
• Home Care/Home Health experience (HCHB)
• Experience with documentation and patient care in a home health setting
