Position Overview
The Hospice RN Case Manager provides comprehensive case management and skilled nursing care to hospice patients in their homes. This role focuses on symptom management, coordination of interdisciplinary services, patient and family support, and end of life care aligned with hospice philosophy.
Schedule & Work Details
- Coverage Areas: Clallam and Jefferson Counties, Western Washington
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours
- On Call/Weekend Requirement: Occasional coverage when night or weekend staff call off
- Mileage Reimbursement: Reimbursed between patient visits only; commute to and from work is not reimbursed
- Pay Rate: $51 to $53 per hour
Key Responsibilities
- Manage a caseload of hospice patients as an RN Case Manager
- Complete comprehensive hospice assessments and ongoing reassessments
- Develop, implement, and update individualized hospice plans of care
- Provide skilled nursing care focused on pain control, symptom management, and patient comfort
- Coordinate care with physicians, social workers, chaplains, aides, and the interdisciplinary hospice team
- Educate patients and families on disease progression, medications, and end of life expectations
- Support patients and families emotionally throughout the hospice journey
- Ensure timely, accurate documentation in accordance with hospice regulations and agency standards
- Maintain compliance with state, federal, and hospice specific guidelines
Required Qualifications
- Active Registered Nurse license in the state of Washington
- Prior experience as a Hospice RN Case Manager
- Strong clinical assessment and case management skills
- Ability to work independently in the field
- Valid driver’s license and reliable transportation
Preferred Qualifications
- Experience with Homecare Homebase (HCHB)
- Strong communication, organization, and time management skills
- Compassionate approach to end of life care
