We are seeking an experienced Home Health Registered Nurse (RN) to provide comprehensive skilled nursing care to patients across a large multi‑county service area. This role is ideal for a confident home health RN who is experienced in OASIS, enjoys clinical variety, and is comfortable managing all visit types while working independently in the field.
Schedule & Work Details- Schedule: Weekdays, 8:00 AM - 4:30 PM
- On‑Call / Weekends: Required participation in weekend and on‑call rotations
- Mileage Reimbursement: Yes — reimbursed between visits, from first to last patient of the day
- Dress Code: Scrubs (any color)
- Perform comprehensive home health nursing visits across the entire service area
- Complete all visit types, including:
- Start of Care (SOC)
- Resumption of Care (ROC)
- OASIS recertifications
- Evaluations and follow‑up visits
- Routine and PRN visits
- Check‑in calls
- Transfers and discharges
- Conduct detailed patient assessments and monitor changes in condition
- Develop, implement, and revise individualized plans of care based on patient needs
- Provide skilled nursing interventions and education aligned with physician orders
- Collaborate with physicians and interdisciplinary team members to ensure continuity of care
- Accurately complete and submit clinical documentation, including OASIS, in compliance with regulatory guidelines
- Participate in on‑call and weekend coverage as scheduled
- Travel daily between patient homes throughout assigned counties
- Active Registered Nurse (RN) license
- Home health nursing experience required
- OASIS experience required
- Strong clinical assessment, documentation, and organizational skills
- Ability to manage diverse visit types independently in a field‑based role
- COS‑C (Certificate of Completion - OASIS‑C)
- High attention to detail in documentation and care coordination
- Strong time‑management skills in a productivity‑driven environment
- Effective communication with patients, caregivers, and clinical teams
