Location: Luzerne County, PA
The Travel Registered Nurse serves as an RN Case Manager for a defined territory, providing 100 percent patient care in the home health and hospice setting. This role is responsible for managing a patient caseload, delivering skilled nursing services, and ensuring continuity of care through routine visits, start of care assessments, and care transitions.
Schedule and Expectations- Weekly Hour Guarantee: 32 hours
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- On‑Call/Weekend Requirement: Every third weekend and one night of on‑call per week
- Mileage Reimbursement: Yes, reimbursed between visits from first patient to last patient
- Serve as RN case manager for an assigned territory
- Perform skilled nursing routine visits in patient homes
- Complete start of care (SOC), resumption of care (ROC), recertification, and discharge visits
- Manage all aspects of patient care within the assigned caseload
- Coordinate care with physicians and interdisciplinary team members
- Provide patient and caregiver education to support treatment plans and goals of care
- Cover visits for fellow case managers during scheduled time off
- Complete accurate and timely documentation in the electronic medical record
- Follow agency policies, hospice and home health regulations, and state and federal guidelines
- Active Registered Nurse (RN) license
- Recent home health experience
- Experience with Homecare Homebase (HCHB)
- Strong time management and organizational skills
- Ability to work independently and collaboratively within a team
