RN – Home Health Case Manager in Lombard, IL
Shift:
- Monday–Friday, 8:00 AM – 5:00 PM
- 40 hours/week
- Weekend rotation: Every 3rd weekend
- No call, no holidays
Location:
- Coverage: Westchester, Cicero, Berwyn, Oak Brook area (IL)
Weekly Gross: $3165
Breakdown:
W2: $45/hr
Stipend: $1365/Wk
Requirements:
- Minimum 1 year Home Health experience (MANDATORY)
- First-time travelers not accepted
- Active IL or Compact RN License
- Valid Driver’s License & Auto Insurance
Certifications:
- BLS
Patient Details:
- Adults (occasional adolescents)
- 5–6 patients per day
Key Skills:
- Patient assessment & care planning
- Wound care, IVs, venipuncture
- Central line & pleural drain care
- Patient/family education
- Time management & communication
Additional Info:
- EMR: Epic (preferred)
- Equipment: IV pumps, Kangaroo pumps
- Orientation: 1–2 weeks
- Dress code: Professional attire
- Floating: Patient homes within coverage area
