About the Role
Alexa Home Health is seeking an experienced and compassionate Registered Nurse Case Manager to join our growing home health team. As an RN Case Manager, you will serve as both a skilled clinician and a care coordinator — visiting patients in their homes to provide high-quality nursing care while managing their overall plan of care.You’ll work closely with patients, families, physicians, and our interdisciplinary healthcare team to ensure the best possible outcomes. This role offers the perfect blend of autonomy, flexibility, and the opportunity to build meaningful patient relationships.

Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Provide skilled nursing services including medication management, wound care, IV therapy, pain control, and disease education.
- Serve as the primary case manager for assigned patients, ensuring care plans are followed and updated.
- Coordinate with physicians, therapists, and other members of the healthcare team.
- Educate patients and caregivers on managing conditions and promoting wellness at home.
- Document care and progress accurately and promptly in our electronic medical record system (Home Health Registered Nurse (RN) – Flexible Schedule)

Requirements:
- Current Illinois RN or LPN license.
- Associate’s degree in nursing required; BSN preferred.
- At least 1 year of Home Health experience (preferred).
- Current CPR certification.
- Strong communication and critical thinking skills.
- Valid driver’s license and reliable transportation.
- Proficiency in electronic charting (Kinnser preferred).
