Pay Range: $40–$45/hour (W2)

Equipment: Microsoft Office Suite, DCMS (Disease Care Management System)
Required Education & Certifications (Mandatory)
• Bachelor’s Degree (REQUIRED)
• No Licensure/Certification Required
Required Skills & Experience (Mandatory)
• Minimum 2 years of experience in healthcare and customer service (REQUIRED)
• Bilingual: English & Spanish (REQUIRED)
• Strong proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
• Ability to navigate multiple systems and web-based applications
• Experience with data entry and tracking systems (DCMS or similar)
• Strong organizational and time management skills
• Ability to prioritize and manage changing priorities under pressure
• Excellent written and verbal communication skills
• Strong problem-solving and analytical skills
Preferred Skills & Experience
• Experience in case management / care coordination
• Familiarity with Medicare MOC requirements and transitions of care processes
• Experience working with PCPs, acute care facilities, SNFs, and home care settings
Job Description / Project
The Transitions of Care Coordinator plays a critical role in ensuring compliance with Medicare Model of Care (MOC) requirements and maintaining continuity of care across healthcare settings.
Key responsibilities include:
• Ensuring plans of care (POCs) are sent to acute care facilities for admitted members
• Notifying Primary Care Physicians (PCPs) of admissions and discharge updates
• Tracking and managing POCs sent and received
• Performing accurate and timely data entry in DCMS
• Collaborating with Business Analysts to generate POC tracking and reporting metrics
• Providing excellent customer service to providers and facility staff
• Supporting coordination between acute care, skilled nursing facilities, and home care
• Performing additional administrative and coordination duties as required
