Schedule: Full time, Monday through Friday, 8 AM to 5 PM
Hours: 40 hours per week
Travel: Yes, between two local long-term care facilities during shift

This role supports a specialized integrated care program serving high-risk members in long-term care settings. The Advanced Practice Clinician is part of an interdisciplinary team providing comprehensive clinical assessments, acute and chronic disease management and advanced care planning. This role offers work life balance with no on-call requirements, no weekends and no holidays.
Key Responsibilities- Conduct comprehensive assessments for members in long-term care environments
- Manage acute and chronic medical and behavioral health conditions
- Document conditions accurately, including ICD-10 coding
- Collaborate with an interdisciplinary care team
- Lead advanced illness and advanced care planning conversations
- Provide education and counseling to patients and caregivers
- Maintain accurate clinical documentation in written and electronic systems
- Active and unrestricted NP or PA license in the state of residence, or the ability to obtain
- Graduate of an accredited Nurse Practitioner (MSN or DNP) or Physician Assistant program
- Active national board certification
- NP: AANP or ANCC in FNP, ANP, GNP or AGACNP
- PA: NCCPA certification
- Active DEA licensure or the ability to obtain
- Ability to lift a 30-pound clinical bag, navigate stairs and perform physical assessments
- Reliable transportation for travel between facilities
- Ability to obtain a collaborative practice agreement if required in the state
- Full COVID-19 vaccination (per federal and client requirements)
- Must pass a pre-employment drug screen
- One or more years of clinical practice experience, ideally in long-term care
- Experience working with patients who have complex medical, behavioral or functional needs
- Background in geriatrics or chronic disease management
- Experience with advanced illness discussions
- Proficiency with electronic medical documentation
- Travel occurs during scheduled work hours
- APC will rotate between two long-term care facilities within Carroll and Galaxy City counties
- No on-call, weekend or holiday requirements
Salary : $65 - $70
