Job Title - Home Health Physical Therapist
Job Type: Contract
Location- Fort Morgan, CO (50-mile coverage area)
Shift: Monday-Friday 8:00 am to 4:30 pm/ 40 hrs/ week
Pay - $54 -$56/ hr on W2
Job Summary:
We are seeking a skilled and reliable Travel Physical Therapist to join our team. This role is focused on performing high-volume blood draws in a clinical setting while ensuring patient comfort, safety, and accurate specimen collection.
Key Responsibilities:
- Perform in-home patient evaluations & develop care plans
- Deliver skilled PT services (mobility, balance, pain mgmt)
- Improve patient independence & safety
- Complete OASIS documentation & visit notes
- Collaborate with interdisciplinary care teams
Required Qualifications:
- MPT or DPT
- Home Health OASIS experience
- Strong clinical & documentation skills
- Ability to travel within the assigned area
Preferred Experience:
- HCHB (Homecare Homebase) experience
