Job description:Job Title: Registered Nurse (Home Health & Hospice)Location: Towson, Maryland, 21204Shift: 8 hoursGuaranteed Hours: 40/weekContract Length: 13 weeksJob SummaryThe Registered Nurse (RN) provides skilled nursing care to patients of all ages in their homes. The RN coordinates care with the interdisciplinary team, patients/families, and referring agencies, ensuring high-quality, continuous, and patient-centered care. The RN also assumes responsibility for care coordination and clinical oversight of support staff.Essential ResponsibilitiesAssess home health and hospice patients to identify physical, psychosocial, and environmental needs.Develop, implement, document, and evaluate individualized Plans of Care (POC).Incorporate physical, psychological, cultural, spiritual, and social data into patient care planning.Maintain accurate, timely, and comprehensive patient documentation.Provide care using proper infection control and OSHA-compliant practices.Educate patients and families on care, medications, diet, home safety, and self-care techniques.Supervise and guide HHAs, STNAs, and LVNs to ensure quality and continuity of care.Assess and manage patient pain; educate patients and families on pain management strategies.Coordinate referrals to appropriate community or clinical resources.Ensure compliance with third-party payer requirements for assigned cases.Participate in on-call rotation and provide timely after-hours support.Communicate effectively with patients, families, physicians, team members, and other healthcare professionals.Attend interdisciplinary team meetings and other required meetings.Perform additional duties as assigned.Knowledge, Skills & AbilitiesAbility to assess and respond to patient/family needs in various home settings.Strong communication skills across all organizational levels.Comprehensive knowledge of medications and correct administration.Proficiency in venipuncture, IV care, wound staging/treatment, inhalation therapy, and phlebotomy.Competence in bowel/bladder training, catheter and drain maintenance, home-use medical equipment, and safe restraint use per physician orders.Strong organizational skills with ability to prioritize and function under stress.Maintains required licensure, certifications, and continuing education.Demonstrates professionalism, growth, and clinical competency.Ability to work independently, collaboratively, and maintain confidentiality.Flexible schedule with ability to travel locally.Required Skills & CertificationsHomecare Homebase experience – RequiredOASIS experience – RequiredCurrent state RN license – RequiredCPR certification – RequiredValid driver’s license, reliable transportation, and insurance – RequiredCHPN certification – PreferredExperience Requirements1–2 years RN experience in a clinical care setting – Required2 years home health experience – RequiredEducationGraduate of an accredited Associate Degree or Diploma Nursing program – RequiredBachelor of Science in Nursing (BSN) – PreferredBest Regards,Tabish Ahmad
