Position Summary

The LPN specializing in Memory & Behavioral Care provides compassionate, person-centered nursing care to residents with Alzheimer’s disease, dementia, cognitive decline, and behavioral health needs. This role supports the clinical team by monitoring resident health, assisting with behavior management strategies, administering medications, and ensuring a safe, structured environment tailored to the unique needs of memory-care residents.


Key Responsibilities

Clinical Care

  • Provide nursing care under the direction of an RN or physician, ensuring adherence to care plans and regulatory standards.

  • Administer medications, treatments, and interventions accurately and safely.

  • Monitor and document residents’ physical, cognitive, and behavioral changes.

  • Perform assessments such as vital signs, weight checks, skin assessments, and monitoring of chronic conditions.

  • Support residents with ADLs as needed during behavior-related or care-complex situations.

  • Maintain accurate and timely documentation in the EMR or paper records.

Memory & Behavioral Care Responsibilities

  • Utilize dementia-friendly communication, redirection, and engagement techniques to support resident comfort and safety.

  • Recognize behavioral triggers, patterns, and early signs of distress or agitation.

  • Implement behavior-support plans in collaboration with nursing, social services, and behavioral health teams.

  • Apply trauma-informed, de-escalation, and non-pharmacological interventions before escalation.

  • Ensure residents are cared for in calm, structured, supportive environments that reduce overstimulation.

  • Monitor the use and effectiveness of psychotropic medications, reporting changes promptly to the RN or provider.

Safety & Compliance

  • Ensure adherence to all facility policies, state regulations, dementia-care standards, and safety protocols.

  • Participate in incident reporting, follow-up documentation, and root-cause reviews for behavioral events.

  • Maintain infection-control practices, including PPE use, hand hygiene, and sanitation protocols.

  • Assist with emergency response drills and safety measures, especially related to elopement or wandering risks.

Team Collaboration

  • Work closely with CNAs, med-techs, activity staff, and interdisciplinary team members to deliver coordinated care.

  • Communicate clearly and professionally with staff regarding behavioral observations and care adjustments.

  • Participate in care plan meetings, shift huddles, training, and performance-improvement initiatives.

  • Support and mentor caregivers on dementia-specific care approaches.

Resident & Family Engagement

  • Provide compassionate, respectful support to residents and their families.

  • Educate families on care routines, behavioral patterns, and dementia progression (within scope).

  • Address concerns promptly, communicating changes in condition with sensitivity.


Qualifications

  • Active LPN license in good standing with the state.

  • Experience in memory care, dementia care, behavioral health, or long-term care preferred.

  • Strong understanding of dementia progression, behavioral symptoms, and safety considerations.

  • Knowledge of de-escalation techniques and trauma-informed care approaches.

  • Ability to maintain patience, empathy, and professionalism in high-acuity situations.

  • Strong documentation, observation, and communication skills.

  • Basic computer and EMR proficiency.

Salary : $30 - $35
