Job Summary
Join our dynamic Wound Care Clinic as a Licensed Practical Nurse (LPN) and Care Coordinator, where you will play a vital role in delivering specialized wound management and comprehensive patient care. In this energetic and rewarding position, you will coordinate wound treatment plans, perform detailed patient assessments, and ensure seamless communication between patients, families, and healthcare teams. Your expertise will help improve healing outcomes, enhance patient comfort, and promote optimal recovery in a fast-paced in-clinic environment. This role has hours of Monday-Friday 8am-5pm with no holidays, weekends, or call!
Responsibilities
- Conduct thorough patient assessments including vital signs, wound evaluations, and medical history reviews using electronic health record (EHR).
- Administer wound care treatments including dressing changes, wound vac changes, as well as advanced wound care healing techniques.
- Support on-site provider during patient clinic times.
- Coordinate wound care plans by collaborating with physicians, specialists, and multidisciplinary teams to develop personalized treatment strategies.
- Manage documentation of patient care activities accurately in medical records, ensuring compliance with HIPAA regulations and ICD coding standards like ICD-10 and ICD-9.
- Provide patient education on wound management, medication administration, nutrition, and lifestyle modifications to promote healing and prevent complications.
Requirements
- Valid and unencumbered Oklahoma or current Compact LPN license required; additional certifications in Basic Life Support (BLS).
- Proven experience in wound care management within settings such as home health, nursing homes, assisted living facilities, or outpatient clinics.
- Previous clinic experience is a plus!
- Demonstrated ability to perform patient assessments accurately while working with diverse populations including seniors.
- Skilled in IV insertion, phlebotomy, medication administration (including injections), catheterization, and specimen collection/processing.
- Excellent communication skills for effective triage and patient education; ability to work collaboratively within multidisciplinary teams.
