Job Title: Wellness Coach
POSITION SUMMARY
The position is responsible for direct oversight of the health and wellness promotion of the ESSHI tenants as well as team members.
ESSENTIAL RESPONSIBILITIES:
Activities include but are not limited to the following:
- Maintains working knowledge of wellness trends to provide effective information and support to tenants.
- Answers tenant questions to support in achieving health and wellness goals.
- Works with tenants and team to identify barriers to wellness
- Encourage the use of ESSHI Learning Lab and provide direct support.
- Encourage use of educational software to support health and wellness as one ages.
- Encourage and monitor use of reading corner by ensuring supplied with reading materials.
- Maintain/check inventory or learning lab and reading corner.
- Plan monthly calendar of events to include health and wellness promotion/educational seminars.
- Encourage ESSHI tenants to attend scheduled events.
- Encourage use of Presbyterian Residential Community wellness gym and track usage.
- Assist ESSHI tenant with developing own personal health and wellness related goals.
- Collaborates with all ESSHI team members for active involvement in ESSHI programing.
- Participates in multidisciplinary team approach to identifying individual goals.
- Provide 1 on 1 and/or group health and wellness goal support.
- Secure and oversee volunteers.
- Willingness to obtain appropriate certifications as designated
- Perform all other duties as directed by the ESSHI Program Manager.
QUALIFICATIONS/EDUCATION/EXPERIENCE
- Bachelor’s Degree in Gerontology, Human Services, Social Services, Psychology or related field preferred, NOT REQUIRED.
- At least 2 year of related experience working with older adults/intergenerational experience.
- Basic knowledge of Microsoft Office (Windows, Outlook, Adobe Acrobat, Word, Excel).
- This position has regular and substantial unsupervised or unrestricted physical contact with individuals receiving services and is required by law under the NYS Justice Center to be fingerprinted for a criminal history record check under OMH (Office of Mental Health).
- Travel is required.
- Must possess the ability to make independent decisions when circumstances are warranted.
- Ability to maintain a professional work relationship with all team members.
Salary : $24 - $27
