Job Title: Care Navigator
Job Overview: The Navigator is responsible for conducting screenings, eligibility assessments, and navigation activities to connect individuals with appropriate health and social care services. This role supports community members by facilitating access to resources that address health-related social needs (HRSNs).
Key Responsibilities:
· Manage incoming referrals and ensure timely outreach and follow-up.
· Verify eligibility and enrollment status using appropriate systems and tools.
· Conduct screenings using standardized tools to assess social needs.
· Determine eligibility for enhanced services and refer individuals to appropriate programs and resources.
· Maintain accurate documentation of outreach, referrals, and member interactions.
· Collaborate with internal teams and external partners to ensure effective service delivery.
· Monitor referral outcomes and escalate issues as needed.
· Use data tools to report trends and support program improvement.
· Provide feedback to leadership on challenges and opportunities for process enhancement.
· Participate in meetings, community events, and other job-related activities.
· Uphold principles of diversity, equity, and inclusion in all interactions.
· Stay informed about relevant policies, programs, and initiatives impacting service delivery.
· Perform other duties as assigned.
Qualifications:
Qualifications & Education:
Associate degree in health, social services, or a related field preferred. Equivalent experience may be considered.
Experience and Skills:
· 5 years of experience in clinical, nonprofit, or managed care settings preferred.
· Strong understanding of health equity and social determinants of health.
· Excellent communication and interpersonal skills.
· Experience with translation services is a plus.
· Ability to work collaboratively across teams and organizations.
· Detail-oriented with strong multitasking and organizational skills.
· Proficient in using technology platforms and data systems.
· Demonstrated ability to meet performance metrics and solve problems effectively.
