As a n Occupational Therapist you will be responsible to:
- Conduct a c omprehensive assessment & evaluation using subjective and objective measures.
- Develop treatment plans and programs in accordance with physician orders .
- Provide quality treatments , actively progressing goals to achieve highest quality of life .
- Conduct screens and provide education to residents /clients , families, caregivers, and staff .
- Coordinate seamless discharge s utilizing the full continuum of therapy and fitness services .
- Provide accurate and timely documentation & billing in electronic medical record.
- Demonstrate understanding and knowledge of payer sources and regulatory requirements.
- Complete and document supervisory visits per regulations.
- Communicate effectively with residents /clients , families , caregivers, physicians, referral sources, and interdisciplinary team .
- Graduate from an accredited Occupational Therapy program.
- Current licensure in the state(s) of practice as required.
- Demonstrated compatibility with PHS's mission and operating philosophies.
- Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
