In-Person | Monday–Friday | Day Shift | On-Call Rotation
We are actively looking for a Spanish/English Bilingual Patient Intake Coordinator to join our growing Home Health team immediately!
Are you fluent in Spanish and English? Do you enjoy helping patients and families navigate healthcare services with compassion and clarity? If yes, we want to hear from you!
This role is essential in ensuring patients transition smoothly from hospital to home care — especially Spanish-speaking patients who rely on clear communication and support.
What You’ll Do:
- Process and review home health referrals
- Verify insurance eligibility and obtain prior authorizations
- Communicate with patients, families, and referral sources in Spanish & English
- Coordinate with discharge planners, case managers, and physicians
- Ensure all required documentation (including Face-to-Face) is complete
- Maintain accurate intake records and referral tracking
- Participate in team meetings and care conferences
You will be the key connection between hospitals, providers, and families — ensuring timely and quality patient care.
What We’re Looking For:
- Fluent in Spanish and English
- High school diploma (Healthcare admin background preferred)
- Strong computer and data entry skills
- Knowledge of medical terminology & insurance verification is a plus
- Excellent communication and organizational skills
- Friendly, professional, and customer-focused
- Valid driver’s license and reliable transportation
Home health or healthcare intake experience is a plus — but we are willing to train the right candidate!
