Deer Meadows Home Health is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our home health team!

This is a great opportunity for a PT who enjoys working independently, building meaningful relationships with patients, and providing quality therapy in the comfort of patients’ homes.

Qualifications

• Current Pennsylvania Physical Therapist license
• Home health experience preferred
• Experience with Homecare Homebase preferred
• Strong clinical assessment and communication skills
• Ability to work independently while collaborating with the interdisciplinary care team
• Reliable transportation and ability to travel throughout the assigned territory
  
  
  

Responsibilities

• Provide skilled physical therapy services to patients in their homes
• Complete evaluations and develop individualized plans of care
• Provide therapeutic interventions focused on mobility, strength, balance, safety, and functional independence
• Educate patients, caregivers, and families
• Coordinate care with physicians, nursing, therapy, and other members of the home health team
• Complete timely and accurate documentation using Homecare Homebase
