About The LPN For Personal Care Role
As a Licensed Practical Nurse (LPN), you will be responsible for:
- Providing high-quality nursing care to residents in our Personal Care Home,
- Developing and implementing individualized care plans in collaboration with the interdisciplinary care team
- Monitoring and reporting changes in resident condition, including any concerns or issues that may arise
- Educating residents, families, and caregivers on care instructions
- Maintaining accurate and complete records of resident care, including documentation of resident interactions
- Must be a Licensed Practical Nurse (LPN) with a current license to practice in the state of Pennsylvania
- Prior experience in a healthcare setting
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
- Excellent communication and interpersonal skills, with the ability to work collaboratively with the interdisciplinary care team
