Huntington Hills Center for Health & Rehab is a 5 Star SNF located in Melville, NY. We are very easily accessible as we are located on the service road of the LIE. Overall, we are a very busy short term rehab and long term care units and you will be treating a blend of both patient populations. We have an extremely spacious rehab gym and a rehab team that excels clinically. We pride ourselves in having excellent patient outcomes. Qualifications

Under the direction of a Rehab Director, a successful Rehab Team Leader will:

• Provide daily oversight of rehabilitation services
• Ensure efficient and effective operations of the facility's rehabilitation department
• Provide therapy services to patients
• Provide clinical support, training and oversight of specialty programs within his/her specific discipline.
• Plan and coordinate with other facility therapists in the implementation of programs and procedures to optimize patient outcomes
• Monitor patient admissions, schedules, MDS assessment periods and data
• Provide administrative supervision of therapists in the rehabilitation department
• Be a licensed Occupational Therapist or license eligible in the state where employed
• Have experience as a treating therapist in the Skilled Nursing Facility setting.
