Job Overview
We are seeking a compassionate and skilled Occupational Therapist or Physical Therapist to join our dynamic team. In this role, you will provide comprehensive therapy services in home health. Your expertise will support individuals of all ages with diverse needs, from early intervention to geriatrics, ensuring they achieve optimal functional independence and quality of life.
Responsibilities
- Assess patients’ physical and functional abilities to develop personalized care plans that address their unique needs.
- Deliver evidence-based therapy interventions aimed at improving mobility, strength, coordination, and daily living skills.
- Assist patients with activities of daily living (ADLs) such as dressing, grooming, and mobility to promote independence.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless patient care transitions.
- Document patient progress accurately and adjust treatment plans accordingly to meet evolving goals.
- Engage in case management activities to monitor patient outcomes and facilitate appropriate referrals when necessary.
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to therapy practices.
- Proficiency in early intervention programs and home health services.
- Ability to develop comprehensive care plans tailored to individual needs across pediatric populations.
- Excellent communication skills for effective collaboration with patients, families, and healthcare professionals.
