Role: Travel Physical Therapist specializing in Home Health, providing outpatient therapy services in patients' homes.
Responsibilities: Conduct regular patient visits (5-6 per day), perform assessments such as start of care, recertification, and resumption of care, and manage patient care within a 50-mile radius. No clinical involvement beyond assessments and therapy, with remote support from clinical coordinators.
Qualifications & Skills: Minimum of 1 year of experience, ND Physical Therapy License, BLS certification, and experience or training in Home Health and OASIS. Valid Driver's License and Auto Insurance required. Ability to work independently in a home setting, managing patient care efficiently.
Job Details: 40 hours/week, Monday through Friday, 8-hour days, no weekends or holidays, with a short orientation period. Scrubs of black, navy blue, or grey are required.
