- Reviewing client referrals and responding to inquiries.
- Performing client assessments to ensure effective home care services.
- Planning, directing, and initiating appropriate in-home care actions independently.
- Adhering to agency policies, procedures, and community standards.
- Performing additional duties as assigned by leadership.
- Graduate of an accredited nursing program; Baccalaureate preferred.
- Current Colorado RN license.
- Minimum two years of nursing experience, ideally in long-term, dementia, or acute care; home care experience preferred.
- CPR certification.
