We are seeking a skilled Home Care RN/Field Case Manager to provide independent nursing visits and coordinate patient care across multiple counties.

- Assessing, planning, and implementing care for diverse patient needs, including wound care, PICC lines, TPN, and tracheostomy management.
- Coordinating with an interdisciplinary team to ensure seamless care delivery and communication.
- Performing documentation such as OASIS assessments, visit notes, and physician orders accurately and timely.
- Supervising home health aides and directing care activities in patients' homes.
- Monitoring and managing complex medical devices and administering infusions.
- Acting as a primary liaison with physicians, caregivers, and families to support patient wellness and independence.
- Current IL RN license, BLS certification, and a valid driver’s license with proof of auto insurance.
- Minimum of 1 year nursing experience.
- Knowledge of Medicare home care benefits, regulatory standards, and documentation protocols.
- Strong case management skills, clinical competence in wound care, central lines, and medication infusions.
- Excellent communication, organizational, and supervisory abilities.
- Providing skilled nursing assessments and education in patients' homes.
- Ensuring compliance with regulatory and documentation standards.
- Collaborating with healthcare team members for comprehensive patient care.
- Managing complex clinical procedures and supporting patient safety and independence.
