RN Care Manager I-IPCM (Full-Time)

Role: The Care Manager collaborates with an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring safe transitions post-hospitalization.

Responsibilities: Perform duties aligned with organizational values, overseeing transition planning and length of stay to enhance patient experience, safety, and quality of care using performance metrics and best practices.

Qualifications:

• Master's in Social Work or completion of all MSW requirements (for Social Workers hired after 12/25/2011).
• 0-2 years of experience in an acute care hospital setting (preferred).
• Current state licensure in Social Work (LMSW, LCSW).
• BLS (CPR) certification required at hire or within 90 days.
• Case Management certification preferred.
