- Teach insulin administration, glucose monitoring technology, and anti-hyperglycemic medications.
- Deliver nutrition and lifestyle guidance tailored to diverse patient populations.
- Assess psychosocial factors and social determinants of health to inform personalized care.
- Conduct on-site classes and remote education sessions as part of a multidisciplinary team.
- Registered Nurse (RN) licensure.
- Strong knowledge of diabetes management, insulin use, and glucose monitoring.
- Excellent communication skills for patient education and team collaboration.
- Ability to assess psychosocial and social factors affecting health outcomes.
- Experience in providing both in-person and remote education sessions.
