Key Responsibilities
- Perform comprehensive assessments of patients' physical, social, and psychological needs.
- Develop individualized care plans collaboratively with patients, families, and healthcare teams.
- Coordinate and delegate care to interdisciplinary teams, ensuring timely and efficient delivery of services.
- Monitor patient responses, evaluate outcomes, and revise care plans as necessary.
- Uphold professionalism by adhering to legal, ethical, and regulatory standards, and maintaining accurate documentation.
- Current RN license in the practice state, with transfer of compact licenses if applicable.
- Basic Life Support (BLS) certification.
- Strong patient care delivery, communication, critical thinking, and team collaboration skills.
- BSN degree preferred, with less than 12 months of experience requiring completion within four years.
- Ability to see, hear, communicate effectively, and manipulate equipment.
- Ability to lift and transfer patients, navigate busy environments, and stand for extended periods.
- May require driving and transportation of supplies and patients.
