Role: Licensed Vocational/Practical Nurse (LPN/LVN) for home health visits

Responsibilities

  • Verify plans of care and deliver nursing visits
  • Document patient care accurately
  • Collaborate with the RN Case Manager to support patient health in home settings

Qualifications & Skills

  • Valid state license as an LPN/LVN
  • Minimum of 1 year clinical experience
  • Ability to travel up to 50% of the time
