We are seeking a Primary Care Physician to deliver equitable, value-based healthcare to Medicare patients in a community-focused setting.

Responsibilities include

assessing and diagnosing patients
overseeing primary care services
prescribing medications
maintaining electronic medical records
collaborating with leadership to meet quality goals
managing care teams with practice managers

The role requires

MD or DO degree
board certification or eligibility in Family or Internal Medicine
active, unrestricted state license
DEA license
US work authorization

Preferred qualifications include

geriatrics fellowship
experience with older adults and value-based care
outpatient primary care experience
bilingual skills where applicable
