Role Overview: This position involves evaluating and treating patients primarily in Rehab and LTAC hospitals, documenting patient progress in the EMR, educating patients, families, and staff, and communicating with team members and external professionals. The role includes preparing equipment orders, contacting vendors, and participating in interdisciplinary team activities.

- Conduct initial and discharge evaluations, set treatment goals, and document appropriately.
- Develop and implement timely treatment plans to achieve functional goals.
- Assess educational needs and provide PRN therapy as needed.
- Collaborate as part of the interdisciplinary team and serve as a mentor for interns if required.
- Participate in skills checks specific to hospital and discipline standards.
- Bachelor's Degree in Physical Therapy required; Doctoral Degree preferred.
- Minimum of 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred.
- Current BLS certification; GA PT license or eligibility.
- Basic computer literacy.
