Occupational Therapist (OT) - Home Health

Role Overview

The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to improve or adjust daily living activities. They collaborate with the healthcare team, modify care plans as needed, and serve as a clinical resource and role model for staff and students. The therapist provides services in patients’ homes within specified regions, ensuring comprehensive assessment, education, and discharge planning.

Key Responsibilities

• Review and communicate care plans with physicians, ensuring timely documentation.
• Educate patients and families, overcoming barriers to learning.
• Document patient visits, assessments, and care plans thoroughly and accurately.
• Update goals and coordinate with interdisciplinary teams and external providers.
• Order supplies, plan discharges, and ensure safe home environments.
• Evaluate patient function, guide therapeutic activities, and monitor reactions to treatments.
• Supervise and instruct Occupational Therapy Assistants, ensuring compliance with regulations.
• Conduct comprehensive assessments, including home safety evaluations and patient scheduling.
  
  

Qualifications & Skills

• Bachelor's Degree in Occupational Therapy.
• Licensed Occupational Therapist in Illinois, BLS certification, valid driver’s license, and auto insurance.
• Ability to work collaboratively within an interdisciplinary team and maintain reliable transportation.
• Strong documentation, communication, and assessment skills.
