Role: Licensed Practical Nurse (LPN) providing in-home care for Pediatric and/or Adult patients.
Responsibilities: Utilize the nursing process to assess, plan, implement, and evaluate patient care; deliver skilled nursing services according to prescribed orders; monitor signs and symptoms indicating physiological and psychosocial changes; collect, analyze, and interpret healthcare data; document nursing diagnoses and patient progress.
Qualifications: Current LPN/LVN license in the practicing state; valid TB or Chest X-ray; current BLS certification; preferred one year of professional nursing experience.
Skills & Key Duties: Strong assessment skills, ability to provide skilled patient care, documentation proficiency, and experience with managing G-tube, tracheostomy, and ventilator care are preferred.
