- Coordinate with interdisciplinary behavioral health and social service teams
- Support person-centered care plans and ensure compliance with clinical and safety standards
- Collaborate closely with families to promote optimal health outcomes in the community
- Licensed Practical Nurse (LPN) licensure
- Strong assessment, clinical, and communication skills
- Ability to perform wound care and patient education effectively
- Proficiency in electronic health record documentation
- Experience working with interdisciplinary teams and community-based care
