Role: The Home Therapy Nurse trains and supports patients in self-care for peritoneal or home hemodialysis, providing quality care and excellent customer service.
Responsibilities: Conducts home assessments, trains patients/care partners, documents care, participates in infection control, manages emergency procedures, monitors inventory, and tracks quality outcomes. Performs routine lab tests, administers medications, and provides on-call nursing services. Coordinates home evaluations for hemodialysis patients and ensures water sampling compliance.
Qualifications: Current RN license, minimum 12 months clinical experience, 3 months in the specific dialysis modality, CPR certification, and computer proficiency. Must complete orientation and demonstrate competency.
Skills: Strong communication, critical thinking, ability to work independently, and knowledge of federal/state regulations.
