Provides clinical leadership to enhance compliance with quality and care standards, managing inter-professional teams and delivering advanced nursing care within regulatory and national guidelines. Acts as a clinical authority, leading care planning, assessments, interventions, and patient education across various settings, including virtual and home visits. Ensures care coordination, quality outcomes, and adherence to healthcare standards such as Patient-Centered Medical Home, HEDIS, and QARR. Participates in team performance improvement, documentation, billing, and peer reviews. May serve as a preceptor and contribute to quality improvement initiatives. Maintains professional licensure and credentials, and supports continuous professional growth.
Qualifications
Includes current RN and Nurse Practitioner licenses in New York, ANCC or AANP certification, graduate nursing degree, and preferred experience in home care or management. Requires knowledge of coding, quality measures, and bilingual skills if applicable. Valid driver’s license may be necessary.
