HOME HEALTH PHYSICAL THERAPIST
Job Title: Physical Therapist – Home Health
Rate: Up to $2650.00 weekly
Location: Marlborough, MA
Day Shift (8H)
Qualifications & Requirements
Education
- Required a degree in physical therapy (Bachelor’s or Master’s)
Experience
- Previous experience in home health care of similar setting is preferred.
- Must have recent Home Health experience.
- Minimum 1 full year Adult Home Health Experience.
- Must have reliable transportation.
- Must have current driver's license.
Licenses/Certifications
- Must hold a valid state license to practice physical therapy
- AHA BLS certification required.
- Must be willing to travel up to 75 miles from home clinic.
Key Responsibilities
- Patient Assessment: Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.
- Treatment Implementation: Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management.
- Education: Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.
- Collaboration: Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs.
- Documentation: Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards.
