Qualifications & Requirements
Education
- Graduate from an accredited school of nursing.
Experience
- Prefer a minimum of two (2) years' experience
Licenses/Certifications
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of New York.
- AHA BLS required.
Key Responsibilities
Patient Care: responsible for monitoring patients' vital signs and overall health. They administer medications, manage pain, and provide treatments to ensure comfort during the end-of-life process.
Emotional and Spiritual Support: offer emotional support to patients and their families, helping them navigate the challenges of terminal illness. This includes providing spiritual care that respects the patients' beliefs and values.
Education and Guidance: educate patients and their families about the disease process, treatment options, and what to expect as the end of life approaches. They guide families in making informed decisions regarding care.
Care Coordination: work closely with other healthcare professionals, including physicians, social workers, and therapists, to create and implement comprehensive care plans tailored to each patient's needs.
Support for Daily Activities: assist patients with activities of daily living (ADLs), ensuring that they maintain dignity and comfort while receiving care.
Crisis Management: trained to respond to urgent care needs, providing immediate interventions for acute symptoms and ensuring that patients receive timely care.
Family Support: support family members by keeping them informed about the patient's condition and providing resources for coping with grief and loss.
