Role Description
This is a full-time on-site role for a Hospice Liaison, based in Fayetteville, GA. The Hospice Liaison will focus on building and maintaining relationships with healthcare providers, patients, and families to facilitate hospice care services. Primary responsibilities include educating the community and healthcare professionals about hospice and end-of-life care, identifying and connecting with potential patients in need of services, and supporting the admissions process. The role acts as a critical bridge between medical providers, patients, and the hospice team to ensure that care transitions are seamless and patient needs are addressed.
Qualifications
- Experience and knowledge in Hospice Care, End-of-Life Care, and Palliative Care
- Proficiency in building relationships and providing support within Home Care settings
- Demonstrated skills in Sales, outreach, or building community partnerships within the healthcare field
- Strong interpersonal, communication, and organizational abilities
- Ability to work in an on-site capacity at our Fayetteville, GA location
- Relevant healthcare certification or degree (preferred)
- Familiarity with hospice policies and regulations is a plus
