Role Description
This is a full-time, on-site Hospice Liaison role located in Fayetteville, GA. The Hospice Liaison will build and maintain relationships with healthcare providers, educate them on hospice services, and ensure a seamless referral process. Responsibilities include collaborating with the clinical team to coordinate care, providing information to patients and families about hospice services, and acting as a key resource for the community regarding end-of-life care options. The role requires proactive communication and strategic outreach to strengthen awareness of the organization.
Qualifications
- Experience and knowledge in Hospice Care, End-of-Life Care, and Palliative Care
- Proficiency in Sales and building professional relationships
- Familiarity with Home Care services and practices
- Strong interpersonal, networking, and communication skills
- Ability to provide compassionate support to patients, families, and caregivers
- Bachelor’s degree in a related field or equivalent work experience in healthcare, social work, or related industries
- Previous experience in hospice, healthcare, or community relations is preferred
- Valid driver’s license and reliable transportation for in-field visits
