About the Role: Community Liaison
As a Community Liaison, you are the first impression of Tapestry Hospice for referral partners, patients, families, and the broader community. Your mission is to build strong, meaningful relationships that support access to compassionate end‑of‑life care.
Key Responsibilities
- Develop and strengthen referral relationships with physicians, hospitals, assisted living facilities, skilled nursing facilities, private practices, and other community health partners.
- Manage a defined territory of accounts while continually identifying new opportunities for outreach.
- Represent Tapestry with professionalism, warmth, and a genuine commitment to patient-centered care.
- Promote our services through strategic marketing, education, and relationship-building activities.
