Job Title: Lead First Steps Coordinator

Reporting Relationship: Compassionate Support Services (CSS) Director

Department: Compassionate Support Services (CSS)

Days Worked: Monday and Thursday

Required Hours: Part-time – 32 hours per week; Hourly / Non-Exempt

Position Summary: The primary purpose of this position is to develop and implement strategies that will maximize staff, volunteer advocates, and client interactions among Compassionate Support Services (CSS) program areas (primarily First Steps), and maximize the transition of PRC medical patients into CSS opportunities. This role will be responsible for sustaining, evolving, and growing with the needs of the First Steps program while connecting client referrals to PRC programs.

Specific Responsibilities:

General:

o Lead the First Steps program with a focus on strategic development, team coordination, and program fidelity

o Represent the First Steps program in internal meetings, including collaboration with Medical Services and other CSS team members

o Support CSS Director in achieving departmental goals and MSIs related to First Steps

o Perform all aspects of intake and processing for referred patients from PRC Medical Services office; process referrals in an efficient and timely manner

o Assist with and track First Steps advocate orientation, including program training for all CSS programs

o Maintain communication and strategy alignment with the CSS team

o Serve as a knowledge leader for local community resources by remaining current on community resources

Client Relations:

o Serve as the escalation point for complex client cases and First Steps advocate support

o Ensure consistency in client engagement and appointment flow across all First Steps advocates

o Maintain oversight of First Steps client case flow, including follow-up communication and documentation of case closure

o Meet with clients when volunteer advocates are absent or unavailable

o Facilitate prompt follow-up with clients in accordance with First Steps program procedures

o Inform CSS Director of problems and concerns relating to the First Steps program

Administrative Duties:

o Oversee client data tracking, trimester enrollment, and program metrics

o Maintain and update First Steps materials, guides, and communication scripts

o Coordinate First Steps advocate schedules and ensure adequate coverage for client appointments

o Assist with First Steps advocate onboarding and training, ensuring alignment with program standards

o Complete day-to-day administrative tasks and operations as necessitated by the position

o Complete and file the appropriate paperwork associated with the position

o Maintain and update texts and contact scripts for client interactions

o Regularly review and revise First Step program guide and materials to ensure relevance and accuracy

o Solicit and submit First Steps program participant photo release forms when applicable

o Oversee text line communication for client interaction and First Steps program enrollment

o Update in-house and external documentation, providing community referrals needed by internal/external program participants, and initiating and maintaining positive

relationships with community providers and referral sources

o Assist with and/or attend PRC events (Gala, volunteer events, Walk for Life, etc.), when directed by CSS Director

o Attend Prayer Summit, all-staff meetings, trainings and other PRC sponsored events, as required

o Perform other tasks as assigned by the CSS Director

Position Requirements:

o BSW or MSW and at least five years of demonstrated job experience in social work; specifically, case management

o Ability to train, oversee volunteerism as exhibited by job experience

o Detail focused

o Excellent organizational skills

o Experienced in cultural competence/emotional intelligence

o Ability to build and retain positive relationships

o Extensive communication (verbal/written) and interpersonal skills

o Willingness to learn, teach, and inform

o Proficient in Microsoft Office, Word, Outlook, and database software

o Knowledgeable in Google documents, forms, and presentations

o Familiar and/or willing to learn Zoom, Google Meets, and TEAMS

o Self-starter, able to work independently in a fast-paced environment

o Physically able to lift 25 lbs.

o Ability to maintain confidentiality while working with highly sensitive data

o Flexible and able to work an occasional evening and/or weekend (additional hours may be required for staff development, during an event season, volunteer assistance, or as requested by supervisor)

o Possess a valid driver’s license as well as reliable transportation (necessary for position requirements)
