This position will work with a brand new clinic in the Eugene, OR area. At the outset, the BHN may provide support across two of our newest clinics. This position has potential to eventually be remote, but will initially be onsite.

What You’ll Do
- Serve as a vital link between patients and behavioral health, substance use disorder (SUD), and social services.
- Maintain a comprehensive database of mental health (MH) and SUD providers, ensuring up-to-date knowledge of available resources.
- Provide same-day support to primary care patients and respond promptly to referrals.
- Be accessible through multiple channels—phone, portal, and in-person—to assist patients and healthcare teams.
- Help patients navigate challenges related to social determinants of health (SDOH), including access to housing, legal services, and community resources.
- Collaborate with care teams, primary care providers, and local organizations to improve patient outcomes.
What You Bring
- Minimum 2 years of college education required; BA/BS preferred.
- Experience in a medical setting is highly desirable.
- Strong interpersonal skills with the ability to build trust and relationships with diverse patient populations.
- Excellent organizational skills and attention to detail, particularly when managing confidential patient information.
- Familiarity with Microsoft Office and electronic health records (EHRs).

Minimum Qualifications
- Commitment to the mission of care coordination.
- Passionate, trustworthy, and empathetic when working with patients.
- Ability to build relationships with different types of people, including patients, organization members, and healthcare providers.
- Strong communication and interpersonal skills with the ability to speak concisely to patients and team members.
- Organizational skills, especially with confidential patient material and appointment tracking.
- Flexibility and adaptability to respond to changing patient and healthcare provider needs.
- Interest in working with patients with chronic health problems and behavioral health needs.
Education and Experience:
- Minimum of 2 years of college education; BA/BS preferred.
- Experience in a medical setting preferred.
- Strong understanding of cultural competency with patient populations.
- Computer literacy and familiarity with MS Office software.
- Exposure to and comfort working in medical settings with medical and psychiatric issues, including issues of death and dying.

General Physical Requirements

Manual dexterity for office machine operation including computer and calculator; stooping, bending to handle files and supplies, mobility to complete errands for deliveries, or sitting for extended periods of time.
